Removal of fibroids that distort the womb cavity may prevent recurrent miscarriages, study suggests

ScienceDaily (Sep. 27, 2011) ? Researchers have found the first, firm evidence that fibroids are associated with recurrent miscarriages. They have also discovered that if they removed the fibroids that distorted the inside of the womb, the risk of miscarriage in the second trimester of pregnancy was reduced dramatically -- to zero.

The study, which is published online in Europe's leading reproductive medicine journal Human Reproduction, is the culmination of 20 years of investigation into recurrent miscarriage by Professor Tin-Chiu Li and his team at the recurrent miscarriage clinic at the University of Sheffield and Sheffield Teaching Hospitals (Sheffield, UK). In addition, for the first time it has given a reliable estimate of the prevalence of fibroids in women who have recurrent miscarriages.

Fibroids in or around the womb (uterus) are benign tumours composed of muscle and fibrous tissue. Although they have been associated with spontaneous miscarriage, until now there has been no evidence of their role in recurrent miscarriages (RM). The prevalence of fibroids has been estimated to be between 3-10% in women of reproductive age, but the prevalence is unknown in women who experience RM, which is defined as three or more consecutive miscarriages.

The researchers analysed data from 966 women who attended the Sheffield RM clinic. The women were scanned for uterine anomalies, including fibroids, via transvaginal ultrasound and radiology, and 79 were found to have fibroids. "This enabled us to calculate that the prevalence of fibroids was 8.2% among women with recurrent miscarriages; this has never been accurately reported before," said one of the researchers, Dr Sotirios Saravelos, who is a clinical research Fellow at the University of Sheffield.

Fibroids were diagnosed and grouped into three classifications:

  • Submucosal -- these grow in the muscle beneath the inner lining of the womb wall and grow into the middle of the womb, distorting the cavity
  • Intramural -- these develop in the muscle wall of the womb and are the most common type of fibroid. They do not distort the cavity and have less than 50% protrusion into the serosal surface -- the outer membrane lining the womb
  • Subserosal -- these grow outside the wall of the womb into the pelvis, do not distort the womb cavity, and have a greater than 50% protrusion out of the serosal surface.

Prof Li used minimally invasive surgery (hysteroscopy) to remove cavity-distorting (submucosal) fibroids from 25 women; 54 women with fibroids that did not distort the cavity had no surgery and they were matched with a control group of 285 women whose recurrent miscarriages were still unexplained after all investigations found nothing abnormal; these women also had no intervention.

In the 25 women who had undergone surgery, miscarriage rates in subsequent pregnancies during the second trimester fell from 21.7% to 0%. This translated to an increase in the live birth rate from 23.3% to 52%.

Dr Saravelos said: "This is the first time that it has been shown that removing fibroids that distort the uterine cavity may increase the chances of a subsequent live birth in women with recurrent miscarriages."

The 54 women with fibroids not distorting the uterine cavity and who had had no surgery also did better after referral to the RM clinic. Pre-referral, the miscarriage rate during the second trimester was 17.6% and this fell to 0% after referral. Live birth rates went up from 20.6% to 70.4% in subsequent pregnancies. This was similar to results from the 285 women with unexplained RM; the second trimester miscarriage rate was 8% pre-referral to the clinic, falling to 1.8% post-referral, while live birth rates increased from 20.6% to 71.9% after referral.

Dr Saravelos said: "These results are interesting because they suggest that the finding of fibroids in women with recurrent miscarriage does not necessarily imply that the fibroids are the only cause of the miscarriage. In addition, they suggest that surgical intervention is not the only means whereby patients with recurrent miscarriage benefit from attending a specialised, dedicated clinic. However, for women with fibroids that distort the uterine cavity, our work shows that removing the fibroids can eliminate miscarriage during the second trimester and double the live birth rate in subsequent pregnancies.

"It has been recognised since the 1980s that women with unexplained recurrent miscarriage have very good pregnancy outcomes following referral to a dedicated clinic without the need for any intervention, and with psychological supportive care, i.e. tender loving care, alone. This usually takes the form of regular visits to a dedicated recurrent miscarriage clinic, regular antenatal scans to check the condition of the baby, reassurance to the mother from the specialist that everything is progressing well and specialist antenatal counselling throughout the pregnancy.

"Interestingly, although women may increase their live birth rate by up to 50% after psychological supportive care, the exact underlying mechanisms involved in this process are not entirely understood. In the present study, the fact that women with fibroids not distorting the uterine cavity do so well, suggests that they also do not have an underlying cause for recurrent miscarriage. As a result, they can also be considered as having 'unexplained recurrent miscarriage', and should be counselled that they have very good chances of a successful pregnancy without the need for any intervention or surgery and with the psychological supportive care offered by a dedicated recurrent miscarriage clinic."

The main limitation of the study is that there was no control group for the women who had their fibroids removed and so it is not possible to tell whether they would have done better without surgery, after referral to the RM clinic. The researchers say that their work highlights the need to perform a randomised controlled trial to investigate this.

"The definitive study requires the recruitment of a rather large number of patients to be randomised between intervention and no intervention. This would require the input of several clinics in a multi-centre randomised controlled trial and its success would depend on the support of all clinics along with that of Sheffield," said Dr Saravelos.

Pakistan frees bin Laden bodyguard

Pakistan recently freed a senior al-Qaida commander who was also an Osama bin Laden bodyguard, raising concerns about the country's commitment to tackling terrorism, media reports say.

Dr. Amin al Haq was freed several weeks ago because Pakistani officials could not prove his ties to al-Qaida and because he is not in good health, according to an Afghan Islamic Press report cited by other news organizations.

Haq reportedly was detained in Lahore by Pakistan's Inter-Services Intelligence agency in December 2008, seven years after escaping with bin Laden, the Daily Telegraph of London reported. Haq served as the security coordinator of Osama bin Laden's Black Guard.

ISI passed Haq, 51, to police in Peshawar in northwest Pakistan before his release, a security source told the Telegraph.

"Amin al-Haq had been arrested mistakenly, therefore, the police failed to prove any charge of his association with Osama bin Laden and the court set him free," he told The Daily Telegraph.

Haq, a doctor, was a member of the Hizb-i Islami Khalis (HIK), a key mujahadeen group that helped defeat the Soviets during the 1980s, according to Mother Jones magazine. HIK also helped bring bin Laden to Afghanistan after he was ejected from Sudan in 1996. And Haq himself reportedly accompanied bin Laden in his escape from Tora Bora in 2001, the magazine said.

Terrorism support alleged
Haq's release follows accusations that Pakistan supports terrorism.

Adm. Mike Mullen, outgoing chairman of the U.S. military's Joint Chiefs of Staff, last week described the Haqqani network, the most violent faction among Taliban militants in Afghanistan, as a "veritable arm" of Pakistan's ISI spy agency and accused Islamabad of providing support for the group's September 13 attack on the U.S. embassy in Kabul.

Secretary of State Hillary Clinton said Wednesday that Washington was close to making a decision on whether to designate the Haqqani network as a foreign terrorist group.

"We are in the final, formal review that has to be undertaken to make a government-wide decision to designate the network as a foreign terrorist organization," Clinton told reporters in an appearance with Egypt's visiting foreign minister.

Also Wednesday, the Obama administration designated two members of the Pakistan-based group Lashkar-e-Taiba as terrorists. The action bans Americans from doing business with the men and blocks any assets they have in the United States.

The Treasury Department identified the men as Zafar Iqbal, a senior leader and co-founder of the al-Qaida-linked organization, and Hafiz Abdul Salam Bhuttavi, who it said has served as the LET's emir on two occasions and was in charge of its day-to-day operations at the time of the Mumbai attack in November 2008.

Indian Prime Minister Manmohan Singh said Mullen's allegations were "something which is nothing new to us. In fact when we were the first to flag this issue earlier, the world didn't believe us."

"Turns out they are disloyal, deceptive and a danger to the United States," Republican Rep. Ted Poe, R-Texas, last week. "We pay them to hate us. Now we pay them to bomb us. Let's not pay them at all."

Pakistan denies the charges.

"We are true friends and we count on each other," Pakistani Prime Minister Yusuf Raza Gilani said in an interview Tuesday.

However, he acknowledged tense relations.

"The negative messaging, naturally that is disturbing my people," Gilani said from his office in Islamabad. "If there is messaging that is not appropriate to our friendship, then naturally it is extremely difficult to convince my public. Therefore they should be sending positive messages."

Rahimullah Yusufzai, a Peshawar-based analyst who has interviewed senior al-Qaida figures including bin Laden, told the Telegraph that Haq's release was a puzzle.

"They could only have released him with the say so of America or if maybe there really was no evidence or he was not that important," he said.

Many cities imposing broad cuts as revenue shrinks (AP)

WASHINGTON ? More than half of U.S. cities have cut staff, canceled construction projects or raised fees this year, according to a report from the National League of Cities that catalogs the vast damage from shrunken property- and income-tax revenue.

Cities are struggling from the same problems that have left the national economy sputtering: high unemployment, a depressed housing market and weak consumer spending. Those factors have reduced the taxes that cities collect for a fifth straight year. Many have had to make up the gap by laying off employees, freezing pay, cutting services, raising fees or suspending building projects.

Two-thirds of city finance officers said they had delayed or canceled public-works projects this year. Two in five reported raising fees for city services. One in five had cut spending on public safety. Nearly one in three had laid off staffers.

"We hoped the worst would be over at this point, but given where the economic considerations are, that seems to be very unclear here in the fall of 2011," said Christopher Hoene, director of the league's research arm and one of the report's authors.

Cities typically suffer the full force of a recession later than states and the nation as a whole do. That's because many cities rely heavily on property tax revenue, which can take several years to fully reflect falling home prices.

By comparison, states rely mostly on income and sales taxes. Income-tax revenue usually falls steeply within months after layoffs. State sales tax revenue also drops as people spend less.

As states' tax collections fell during the recession, they responded by cutting aid to cities, school districts and localities. Those cuts are expected to peak next year, according to research by the Center for Budget and Policy Priorities cited by the league's report.

Public education is especially hurt because many school districts are funded about half by states and half by property taxes, said Michael Leachman, the center's director of state fiscal policy.

"The bursting of the housing bubble has now caught up with property tax revenues, so that's making it harder for local governments to offset the declines in state aid that they've been seeing," Leachman said.

In Cleveland, the school board was to vote Tuesday night on whether to lay off teachers for the second time this year. The school district says it would have to find more than $10 million in cuts to help balance its budget and save the jobs of more than 300 teachers.

This past summer, the city laid off 319 employees, including 81 police officers.

"Cleveland was chugging along," said Andrea Taylor, press secretary for Mayor Frank Jackson. "It wasn't as if we had a big surplus, but we were managing our share of the budget crisis well. We weren't set to have any layoffs at all."

But she said Ohio's recently enacted budget slashed state aid to the city by 25 percent in fiscal year 2011 and 50 percent in 2012. The budget was adopted in late June and enacted in July ? a timeline so quick that Cleveland couldn't react without laying off workers, Taylor said.

The combination of state cuts and falling tax revenue amounts to a "double-whammy" for city residents that's likely to persist for years, said Scott Pattison, executive director of the National Association of State Budget Officers.

"States really are not in a position to go back and restore those cuts," Pattison said. "Money is so tight, they don't have it to start giving it back."

Over time, the cuts tend to dampen growth by making companies reluctant to spend and hire, said Mark Vitner, senior economist at Wells Fargo Securities.

"If it becomes tougher for business to get done because of government cutbacks, or if businesses have a harder time finding skilled workers as the education system crumbles, it can create a nasty feedback loop that can go on for years," Vitner said.

In South Carolina, cities and counties are receiving 35 percent less from the state than in the 2008-2009 budget year. To close the gap, Clinton, S.C., had to cut 10 workers from its roughly hundred-person payroll this year. Three of the 10 worked in public safety.

Interim city manager Frank Stovall said Clinton can still provide basic services. But he said some services like recreation and community policing had been cut and utility rates increased.

"We have a plan to build bike paths and green spaces and parks," Stovall said. "It's been sitting on our shelf now since 2003 with no hope of implementing it."

In the Columbia, S.C. suburb of Cayce, garbage collection has been reduced from twice to once weekly. And Greer, S.C., near Greenville, has eliminated mosquito spraying and reduced leaf collection pick-ups.

The hardest-hit cities have been those that depend most on income from property taxes, the report said. Many are in the Northeast, Hoene said. By contrast, Midwestern cities tend to have steeper income taxes. And cities in the West, South and Southeast typically rely more on sales-tax revenue.

All three categories of tax revenue are expected to decline in 2011, Hoene said.

The report is based on a survey taken this spring and summer, before worsening economic data and fears about Europe's debt woes hurt consumer confidence and caused wild swings in the stock market.

"There may be a reconsideration of that as this year continues to unfold," said another co-author, Michael Pagano, dean of the College of Urban Planning at the University of Illinois and Chicago. "We're looking at at least another two or three years of very troubling fiscal signals for cities and municipalities."

The report is based on an annual survey of finance officers from cities and municipalities with more than 10,000 residents.

NEW YORK ? Justin Timberlake has been lauded for his work in music, TV and film. Now he's about to get an accolade for trying to better the environment.

The Environmental Media Association says Timberlake will receive its Futures Award, which represents future environmental leaders in entertainment.

The group cited Timberlake's attempts to reduce his carbon footprint during his tours, his advocacy for environmental issues and his eco-friendly golf course outside of Memphis, Tenn.

EMA President Debbie Levin called Timberlake "a great example of how you can lend your voice for positive change."

Others who have received the award include Edward Norton, Rosario Dawson and Maroon 5.

The Environmental Media Awards will be held Oct. 15 in Burbank, Calif.

Cops: Couple's copper theft for wedding netted $18

(AP) ? Police say a western Pennsylvania couple desperate for money to pay for their wedding netted just $18 for the stolen copper wire they cut from more than a dozen utility poles.

North Sewickley police say 23-year-old Joseph Russell and 24-year-old April Cater cut down the wires on August 9, four days before their wedding.

Russell says he was desperate for money because he'd just lost his job and lost a $1,000 deposit after his reception hall abruptly closed down.

Sgt. Jeff Bezce says the couple clearly expected to get more money than they got from the scrap where they sold the copper.

Russell tells WPXI-TV he will pay for the damages. Utility officials say it will cost about $400 to repair the 18 poles that were damaged.

BANGKOK ? Amnesty International is calling on Muslim guerrillas in Thailand's southern border provinces to stop targeting civilians.

The London-based rights group says that noncombatants have accounted for two-thirds of the nearly 5,000 people killed since insurgents took up arms in 2004.

Bomb blasts or shootings are reported every few days in the far south, and the London-based rights group says many of the attacks constitute war crimes that should be prosecuted under international law.

Amnesty said in report Tuesday that at least 4,766 people have died and 7,808 have been wounded in the conflict.

Five found dead in rural Indiana, police say

Five people were found dead on Sunday afternoon in two rural southeast Indiana homes after police were called to check on a small child wandering near the road.

Some of the five had been shot, and autopsies to confirm the cause of death are planned for Monday, state police said.

Police said it was not clear if any suspects remained on the loose or were among the dead, and warned residents of the sparsely populated, wooded area near the town of Laurel to be careful.

"We have five dead people and we're trying to find out how they died," said Sgt. Jerry Goodin, spokesman for the Indiana State Police. "Some of the victims had gunshot wounds, or what appear to be gunshot wounds."

The Marion County coroner's office in Indianapolis was scheduled to perform autopsies Monday.

Police did not immediately release the identities of the victims, pending notification of family members. Goodin said Sunday he did not know "if the shooter or shooters could be among the deceased."

Lone child found wandering
A multi-agency investigation into the killings began after the Franklin County Sheriff's Department responded to a call Sunday afternoon about a child wandering along a road near Laurel, about 50 miles southeast of Indianapolis, according to a state police statement.

A deputy spoke with the child and neighbors, then called for backup to search the two homes. Police found a dead male inside a recreational vehicle and the bodies of two males and two females at the other home across the street. The statement didn't say if the dead were adults.

Goodin told The Associated Press that it was too early in the investigation to rule out the possibility that one or more suspects were on the loose and cautioned neighbors until that could be clarified.

"They should be vigilant," Goodin said. "They need to make sure they keep the doors locked. If they see any suspicious people, absolutely they should dial 911.

President Barack Obama stressed the importance of quality education in a speech Friday when he announced that any states willing to commit to higher standards of education can get a waiver from the No Child Left Behind law.

Obama said the measure, introduced by President George W. Bush in 2002, had ?admirable? goals but that, in practice, ?teachers are being forced to teach to a test,? and that important subjects like science and history have fallen by the wayside.

Obama also emphasized that improving and investing in America?s public schools is an important step to economic recovery. He promised that his jobs bill, should Congress pass it, would update 35,000 schools and put thousands of teachers back to work across the country. ?BF

MIAMI (AP) ? Endurance athlete Diana Nyad pressed on despite painful jellyfish stings and kept swimming treacherous ocean currents beneath the stars early Sunday, about halfway through a grueling course from Cuba to Florida.

As of the latest update before sunrise, her crew said the 62-year-old swimmer had completed 49 miles (79 kilometers) of the 103-mile (166-kilometer) passage of the treacherous Florida Straits and was swimming beneath a nighttime sky bristling with stars. If all goes well, she hopes to complete her sea swimming odyssey Monday morning.

Updates have been coming from her handlers via Internet posts, and already Nyad has been in the water longer than her aborted attempt at a Florida Straits crossing last month, when she lasted 29 hours before a crippling asthma attack forced her to call it quits.

Her latest attempt also has been fraught with difficulties.

Nyad suffered a weekend jellyfish sting that painfully affected her face and eyes and was treated late Saturday aboard a support vessel. But her team tweeted early Sunday that she hit the water again and was continuing her methodical strokes toward her goal. Her website said she had completed more than 34 and a half hours in the water soon after 4:30 a.m. EDT (0830 GMT) Sunday ? or more than 93,000 strokes.

An independent observer said she could still pursue her attempted record-setting swim from Cuba to Florida despite seeking treatment.

Nyad's team said early Sunday in a post on her website that the record would still be valid as long as she was on the boat only for treatment and not to rest. It didn't elaborate on the specific treatment she received or how long she was out of the water, but a team blog update posted before dawn said she resumed swimming at the "exact spot" where she had paused.

"The rules say she is now going for a record staged swim rather than a non-stop swim," the updated noted.

Her organizers said making it through the second night would be a key test to completing her swim as she struggles with the physical and mental challenges of such a distance. Her website said she was breaking up the swim into manageable blocks of time and getting through difficult nighttime hours by recalling favorite songs and thinking upbeat thoughts.

"Tonight, her second night in the open water, may be the most critical," Nyad's website said.

The latest blog update before dawn included a photograph of the stars and said "for now, no news is good news" as she took aim for the Florida Keys.

LONDON (Reuters) ? What name would you give to a species of Antarctic, sea-dwelling worm that spends its time 2,000 metres below the water's surface, wriggling in the rotting carcasses of whales?

The British public will get to name five newly discovered species of this deep-sea worm, the Natural History Museum in London said on Friday, as it opens its doors to explain that taxonomy -- the practice of naming new species -- is not taxing, but fun.

"Our goal is to show that taxonomy, the scientific discipline of naming new species, is interesting, fun and crucial to the advancement of science," zoologist at the National History Museum, Adrian Glover, said.

The unnamed worms are known as annelids, a group that also includes the familiar garden earthworm, as well as more unusual critters such as the giant hydrothermal vent tubeworm.

Deep sea annelids are an incredibly diverse group and scientists believe they perform vital recycling of nutrients on the seabed.

"These five species of worm are ecosystem engineers that hoover up the ocean floor, which is very important for the ecosystem. We know virtually nothing about these worms -- they don't even have a name yet," Glover told Reuters.

The scientific names given to animals must be comprised of two parts -- the genus and the species -- and must have a Greek or Latin origin.

The genus for these five worms is Ophryotrocha, and scientists will ask visitors to the museum's event, "Science Uncovered," to examine the specimens and come up with a name for each species.

Taxonomists often consider the shape, pattern, or location of origin of a creature when deciding on a suitable name.

The worms waiting for names were discovered in a variety of marine habitats, including volcanic vents on the seafloor and even rotting carcasses of dead whales -- sources of nutrients in a vast underwater environment where food is scarce.

"The names don't have to be serious, but they have to be the unique. Although the genus is fixed, you can be a bit more imaginative with the species," Glover said.

"For example, we discovered a group of worms in the Pacific Ocean that, when grouped together, looked like the Greek, shaggy pile carpets you can buy in IKEA, so we named it "flokati,"" he said.

Some scientists have even looked to showbusiness for inspiration in naming new species.

Calponia harrisonfordi is a type of spider named after Harrison Ford, to thank the Hollywood actor for narrating a documentary for London's National History Museum.

Musician Frank Zappa also has a spider named after him, Pachygnatha zappa, because its unique markings resemble his famous mustache.

"It provides a link between species, so biologists can communicate and know they are working on the same animals," he said.

"There are strict rules to taxonomy, and we want to explain how it happens and how we link it to specimens in museums. What is crucial is that the name is unique and backed up by data, such as DNA sequences, fundamental descriptions and specimens," he said.

The London National History Museum collection alone contains roughly 70 million specimens, many of which are smaller animals such as worms, Glover said.

With scientists yet to discover about 90 percent of the estimated 9 million plant and animal species on earth, according to a recent study, this collection of jarred curiosities can only increase with future exploration of the deep seas.

"We don't even know what's down there, let alone what role it plays in the ecosystem," Glover said.

"Exploration of the deep sea is akin to what rainforests were to scientists about 200 years ago, where they discovered lots of new species every day," he said.

"And the first step to take when you find new species is naming them."

The free event, "Science Uncovered," takes place on Friday, Sept 23 from 4-11pm.
